The Injury Recovery & Sports Performance Patient-Provider Contract
​We Commit To: 
  • ensuring that you experience safe and positive care at every appointment​​​
  • providing the best possible care through our relentless drive to learn about and optimize human biomechanics, injury recovery, and sports performance
  • helping you achieve your injury recovery and sports performance goals as agreed upon at your first visit (some of you may have a lifelong goal to simply move and feel better, and we can help you maintain that goal indefinitely​)
You Commit To: 
  • a specific frequency of appointments over an agreed upon duration (infrequent appointments and no shows means it will be impossible to help you achieve your goals)
  • filming yourself performing home exercises we will review these videos prior to or during your appointments
  • performing the home exercises as prescribed
  • actively participating in achieving your goals
  • optimizing your nutrition and sleep habits 
  • providing film and notes about the movements and positions that hurt and/or hinder your performance
  • providing a time lapse video of you sitting in your most frequently used chairs/couches while at work or at home (if you only stand while working, we need video of that too​)
You Also Commit to Being Relentlessly Optimistic: 
  • success should always be rooted in a positive mindset (believing that you will get better is an important mental strategy to actually getting better)​
  • not every recovery is linear which means you must agree to be patient while your body adapts to new movement patterns and believe in the process (we're available by email, text, and phone to help anytime questions or issues arise)​​​
